OUR IMPACT
2024/25 at The Creighton Centre has been a year of connection and care. From supporting older people at risk of loneliness, to creating learning and social opportunities for adults with learning disabilities, to helping people live safely and independently in their homes, everything we do is about improving the lives of the some of the most vulnerable and providing a welcoming space for everyone in our community.
We’re proud to share the highlights of what we and our team of incredible volunteers achieved together in 2024/25. Scroll down or click any of the icons below to jump to a section of our report.
BY NUMBERS
1,672 people joined in at our Community Centre through activities and events.
102 adults with learning disabilities supported to build skills, friendships and independence.
187 vulnerable households made safer with 600+ pieces of safety and security equipment installed.
There were 17,900 attendances at our community centre throughout the year.
85 older people supported after illness or falls through Keep Active – plus 37 new exercise classes with 264 attendances.
160 homes adapted across London through Care & Repair, helping people to remain in the homes that they love.
345 older neighbours connected through Homeline:
- 12,500 phone calls made
- 2,000 event attendances
- 118 Christmas hampers delivered
- 8,400+ volunteer hours given
Our incredible team of 120 volunteers donated over 10,000 hours of time, skills and care to our clients.
COMMUNITY CENTRE
Our Community Centre buzzed with life this year, welcoming over 1,600 local people to 74 different classes, events and activities – everything from art and dance, to peer support groups and health & wellbeing and learning opportunities. Our Centre is a place where connections are made and community grows, providing an accessible space for residents and community groups across the Borough. Plus our building provides affordable office and workshop space for a wide range of charities, social impact and well being organisations who help our community to thrive.

LEARNING DISABILITY OUTREACH (LDO)

Our LDO team supported 102 adults with learning disabilities, helping them build skills, friendships and independence. With the support of a team of brilliant volunteers, we ran 200 events and workshops where participants enjoyed socialising, mentoring, literacy, health sessions, art and singing classes and even gardening in our community space.
Our staff also enabled our participants to achieve their personal independence goals though many hours of one to one casework, coaching and advocacy.

"I do lots of great things with the project, which I really enjoy. I do yoga, gardening, literacy lessons, tai chi to name a few. Everyone is really helpful, it is a very friendly place to be."

The project helps me to volunteer as a receptionist. I love helping the community when I am volunteering – I really like what I do!

HOMELINE
Homeline continued to be a lifeline for isolated older people, reaching 345 members with daily calls, home visits, walking support, social events and casework support.
This year we also launched our new Dementia Circle of Care funded by the Charity of Sir Richard Whittington, creating home/care home dementia trained befrienders, inclusive Centre based activities and stronger support networks across the borough.

KEEP ACTIVE
In partnership with Hammersmith & Fulham NHS Rehabilitation Services, we supported 85 older people to regain strength and confidence after illness or a fall helping them to build their ability to reconnect with their social support networks and the wider community. Plus we launched a new Strength & Balance class for over-60s, supporting 44 individuals with 600+ attendances this year. And thanks to new funding from LBH&F’s Active Minds programme we were able to launch weekly line dancing classes, bringing joy, movement and social connection to 35 individual attendees during the year.

Case Studies:
Eleanor: Regaining Confidence on Six Flights of Stairs
Living alone on the 5th floor with no lift access, Eleanor faced debilitating anxiety and frequent falls. Her flat had become a self-described “dungeon.” Initially unable to go outside alone, Eleanor’s sessions with Keep Active helped her slowly build confidence. Though a panic attack struck during her first outing, she persisted and eventually navigated the stairs independently to attend a dentist appointment. Ultimately, she was re-referred to the CIS for further equipment and support to access community —testament to Keep Active’s role in bridging gaps after NHS discharge.
Jerome: From Isolation to Independence
Jerome was referred to Keep Active with the goals of walking outdoors with his 4 wheeled walker and accessing the community. Jerome, living with severe COPD and depression, relied on his weekly Keep Active sessions as his only motivation to leave home. His Keep Active sessions supported him to keep walking and continue accessing the local shops to maintain his independence.
National Recognition
We were delighted that Keep Active was featured in an EMD UK campaign promoting the benefits of exercise for older adults to the government. This recognition highlights the importance of local services like ours. Frankie Leibe, a former volunteer, now leads our expanding Stay Active classes. EMD UK praised her leadership in creating a safe and enjoyable environment.

SAFER HOMES
We made homes safer for 187 households – fitting over 600 pieces of safety and security equipment, from fire alarms to door locks, and supporting families with young children, older residents and survivors of domestic abuse to feel safer and more secure in their homes.
- 33 Adult Assessments carried out
- 60 Child Assessments carried out
- Over 500 pieces of safety/security equipment fitted
- 58 small jobs carried out
- 43 Home Essential Visits carried out
- Supported 191 clients
CARE & REPAIR
We completed home adaptations for 100 clients across eight London boroughs – from installing ramps and grab rails to managing bigger projects such as walk in showers and stairlifts. Our team makes sure older and vulnerable people can live safely and independently, even during major changes to their living spaces, enabling them to remain at home for as long as they can.

“We are so pleased with the work done in our bathroom. The ergonomics have improved significantly. Barbican bathrooms are so small that everything is a tight fit. But the job done to rearrange the shower mixer and grab rails allows us more space to shower in safety. The mini heater is a huge blessing as the underfloor heater does not produce enough to make the bathroom comfortably warm. The workmanship is excellent. Thank you so much”
