How we help
Homeline offers long-term befriending services, helping to reduce isolation and loneliness for residents aged 60 and over living in Hammersmith and Fulham.
We make weekly phone calls to our members for a chat, to check all is well and assist with any problems. We offer home befriending and match our most isolated members with a volunteer for frequent home visits or support getting out and about. Our team of staff can offer advocacy and casework support. In addition, we run regular classes and social events, a great opportunity to get more involved in your local community.
Most recently we have expanded our service to support more people living with dementia at their home and in care homes.

Homeline support
Telephone befriending
We make weekly befriending phone calls to our members for a chat, to check all is well and to assist with any problems. Local information, signposting and referrals can be made to appropriate services. When we can’t reach a member we carry out safe & well checks to make sure all is well.
Volunteer befriending
Home visits
We have a team of friendly volunteers ready to provide home befriending support to residents. Homeline matches our most isolated members with a volunteer for frequent home visits or support getting out and about.
Volunteer befriending
Outdoor walks
We can match residents with a volunteer for walking. This service is for residents who can walk independently but wish to build confidence in accessing the local community and/or simply stay healthy. Enjoy a walk to a local cafe or class.
Regular Classes
Social events/lunches
Currently we have IT, line dancing, knitting, exercise and reminiscence classes on offer to Homeline members at The Creighton Centre. In addition, we run monthly social events – a great opportunity to get more involved in your local community and make new friends.

In addition, we organise information workshops throughout the year on a variety of health and wellbeing issues affecting our members.
Dementia support
Circle of care for residents living with dementia
Specially trained volunteers offer weekly visits and meaningful activities at home, while our Centre hosts inclusive sessions such as nutrition workshops, line dancing, reminiscence and gardening. We also work to support carers and raise awareness across the community, helping to ensure that those living with dementia can stay connected, engaged and supported.
Keep Active
Keep Active is a volunteer delivered integrated care service, run in partnership with the local NHS rehabilitation teams. Keep Active helps older people aged 60 and over back to independence after a fall or illness. Volunteers are matched with clients and visit them weekly for up to 9 weeks, providing encouragement and support to continue working on their mobility goals. This could include helping with an indoor exercise programme, accompanying someone outdoors, helping with stairs or practising a transport route to an exercise class.
The service bridges that important gap between a period of intense healthcare input and having the confidence to access the community once more, reducing NHS readmissions.
The service is free to people aged 60 and over living in the borough of Hammersmith & Fulham who have been referred to us via local NHS community rehabilitation teams and other health teams.
Volunteers are DBS checked and are specially trained by rehabilitation therapists.
9 week volunteer support programme
Older people 60+ discharged from Community Rehab Services are matched with a Keep Active volunteer, who provides support and encouragement with indoor mobility exercises and outdoor walking through 9 weekly visits.
Stay Active exercises classes
Fun and inclusive exercise classes designed to help people maintain their fitness, balance, strength and flexibility. Open to residents aged 60 and over living in Hammersmith and Fulham.

Day/time: Class 1 Thursday 10:30 – 11:30
Class 2 Thursday 12:00 – 13:00
